Therapy that fits into real life
Skills stick when they're learned where they're used. We bring everything to you — and build progress into your child's actual day.
We come to you
No packing up a tired kid for a clinic. Therapy happens in the place your child is most comfortable and most themselves.
One therapist, every visit
Consistent, personal care from the same licensed OT — not a rotating cast. We get to know your child and your family.
You're part of the plan
You'll get simple, doable strategies to use between visits, so the progress keeps going all week long.
Real skills, built where they're used
Every child is on their own trail. We meet them where they are and help them take the next step — through play, not pressure.
Handwriting & fine motor
Pencil grip, using scissors, buttons and zippers, and the hand strength behind it all.
Sensory & regulation
Big feelings, sensitivity to sound, touch or movement, and tools to help your child stay calm and ready.
Everyday independence
Dressing, feeding, brushing teeth — the daily routines that build confidence and self-reliance.
Focus, play & coordination
Balance, body awareness, and staying engaged — so play and learning come a little easier.
School & home support
We work with whatever comes up at school or home, plus simple coaching so you can keep the momentum going.

Care from a real, licensed OT
Little Trailblazers was founded by Lexy Mouannes, a licensed pediatric occupational therapist who believes kids do their best growing in the places they already feel safe.
That's the whole idea behind "grow as you go" — meeting your child on their own path, at their own pace, and celebrating every step forward together.
"Kids do their best work where they feel most at home — surrounded by their own toys, their own routines, and the people who love them. My job is to meet each child exactly where they are, make the work feel like play, and leave families with simple tools they can keep using long after I go. Every small step is worth celebrating." — Lexy Mouannes, OTR/L · Founder
Grow your career on the trail
We're building a team of caring, curious pediatric OTs who believe therapy works best where kids actually live and play. Even when we're not actively hiring, we love connecting with great clinicians.
Who we're looking for
Licensed pediatric occupational therapists who are warm, family-centered, and excited about in-home, play-based care across San Diego.
Why you'll love it
Flexible scheduling, real one-on-one time with each child, supportive mentorship, and a small practice that genuinely values your clinical judgment.
Let's connect.
Whether or not we have an opening right now, send us a little about yourself and your résumé — we're always glad to start a conversation.
